Insurance Benefits Check If you’d like us to check your insurance, please make sure you fill out the correct insurance info in the new patient intake form or submit the insurance info to us (Please send us photos of the front/back of your insurance card and your ID) at least 72 hrs before your appointment. Otherwise we cannot guarantee that we can get your benefits back before your appointment. Thank you for your cooperation.
LATE CANCELLATION PERIOD Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the practitioner’s day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee (full price of the missted appointment) to the card on file. Please contact us ASAP if you need to reschedule your appointments to avoid late fees. Thank you for your understanding!
